Cant add Form to Patient->Visit

Description

When editing a Visit, trying to add an item "Form" results in a Visit item which says;

[date] [form] No Document
eg.
27/01/08 form No Document

Matt, I cannot repeat this in version 1.1-beta1 and also ther eis not enough information to reproduce this behaviour.

Did you enetr a form ?
Did you add a description ?
Is this version 1.1-beta1 or your current version 1.0 ?

Sorry about the delay in responding to this issue Tony;
This behaviour is occurring in Version 1.0 as of 03/03/08.

In Patients->Medical Records
1. Create a visit.
2. In visit edit dialog add a "form" using the drop down box.
3. Complete the description (eg.Test 080303) and choose a form template.
4. Press OK.

The form is now visible as a document within the Documents tab but within the medical record the entry appears as follows;

eg.
03/03/2008 Form Test 080303 No Document
